Complete the Online Intake Form

Once your appointment is booked, please click the link in your confirmation email to fill out the Intake Form prior to arriving for your first visit.

Arrive On Time

Please arrive on time for your scheduled appointment, and your practitioner will promptly receive you at the reception area.

Dress Comfortably

Please wear loose fitting clothes that can be rolled up past the knees and elbows, as you will be receiving your first acupuncture treatment during your first visit. In the event that you are unable to dress comfortably, we can provide you with a hospital gown to wear.
